REGISTERED NURSE - Lake Charles Community Based Outpatient Clinic (CBOC)

Major Duties to include but are not limited to:
Complies with standards, policies, and protocols of hospital, state and federal organizations. Strong and effective communication skills to facilitate patient care and safety, while maintaining the role of patient advocate. Adapt to any vital role of the healthcare team as needed. Appropriately handles high-pressure situations along with Assessing, intervening and stabilizing with decisive action. Determines priority of care based on physical, psychological and social needs. Direct patient care includes scheduled clinic visits, walk-in visits, urgent visits, Patient group visits, telephone calls and Video on Demand (VOD) clinical visits. Demonstrates completion of nursing clinical reminders, follows up on all clinical tasks including all communication with Patients and VA Team Members in a timely manner. Applying the Patient Aligned Care Team (PACT) model of patient care, to Assess, plan, implement, and evaluate patients' care processes. Using the PACT process, recognizes specific patient needs and goals and reports important findings and/or changes to the Provider. Performs focused subjective and objective assessments of patients as well as surroundings. Collaborates with professional peers, members of the interdisciplinary PACT team, and the programs/services and community resources in meeting patient care needs. Reports significant changes to provider, Identifies specific patient goals, using the PACT process. Identifies high risk populations. Initiates patient centered care based on assessment and reassessment during each patient encounter based on evidenced-based guidelines. Provides psychosocial support to patients/family/care givers through telephone care, direct care and support services and documents care provided in medical record accurately. Communicates appropriately with the patient and family members at level of understanding to establish a continuous healing relationship for health promotion disease prevention, acute care, chronic care and end of life care. Provides patient and family education. Triages to other team members as appropriate, follow-ups. administer treatments and medications. Utilizes resources for additional support for patient care. Demonstrates completion of clinical reminders assigned to nursing. Complies with Continuous Education requirements mandated by the VA and State Licensures. Ensures and maintains patient and employee records and confidentiality at all times. Work Schedule:

Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The population of male and female veterans served ranges from the adult, age 18, to the elderly adult, age 65 and over. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required up to 50 pounds. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. In addition, the incumbent must be emotionally stable and adjust to different types of personalities, demonstrating cheerfulness, compassion, courtesy and concern. He/she must be quick and dependable in taking emergency action as directed and use initiative in preventing accidents.

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
